Please Read Before You Submit

This Form is for requesting alternatives/evaluations for courses housed within the Departments/Schools in the College of Fine Arts and Communication.

Students may submit this form to request the following:

  • Alternative Course Request - Requesting an alternative course in place of a specific degree requirement.
  • Course Evaluations - Requesting evaluation of a non-direct equivalent transferred course for possible credit towards your degree.

When you submit a course alternative/evaluation request, your academic advisor will review the request and forward it to the appropriate department/school representative for a determination. Be aware that advisors do not make the final determinations regarding approval/disapproval of course alternatives/evaluations.

General Education Courses

If you are requesting evaluation of previously completed General Education (Core) Courses, you must complete the separate Transfer Evaluation Request.

Non-Fine Arts and Communication Courses

If you are requesting course substitutions/evaluations for Non-Fine Arts and Communication Courses, please utilize the appropriate College/School/Departmental Forms. If you are unsure of where to submit a request, please contact your advisor for guidance.
